Services

Level I – Therapeutic Care
This service is designed for consumers consistently independent with activities of daily living (ADL), however needs daily reassurance and coaching.  Requires minimal observation in medication management.  Planned activities and group sessions are implemented to assist the consumer in developing more appropriate relationship skills.  Job search assistance and interview training is provided to teach consumers basic skills needed to obtain employment.

Level II –  Reinforced Therapeutic Care
This service is designed for consumers capable of activities of daily living (ADL), however negligent in their responsibilities.   Requires more verbal supervision and coaching with reinforced consequences for behavior.  Focus is broadened to include staff intervention with decision making and assist with services as needed.  Job search assistance and interview training is provided to teach consumers basic skills needed to obtain employment.

Level III --  Person-Centered Therapeutic Care
This service is designed for consumers in need of Treatment and/or Behavioral Support planning prior to admission.  This consumer requires a structured environment with constant supervision and consistent reinforcement of consequences for inappropriate behavior.  Planned activities and group sessions are implemented to assist the consumer in developing more appropriate relationship skills.  Individualized one on one support in developing daily life skills with weekly documentation provided to families and/or guardian.   Written, supportive correspondence to courts, probation officers, therapists and family members for mandated compliances.

Level IV –  Individual Therapeutic Care
This service is designed for consumers  in need of therapeutic care with an  emphasis on individualized interventions for specific skill acquisition.   Treatment plans will be developed to help the consumer master their obstacles in achieving or maintaining the highest level of independent functioning.  Consistent reinforcement of consequences for inappropriate behavior.  Daily documentation of progress  is provided and written correspondence to courts and probation officers for mandated compliances, as needed.  A higher level of consultative and direct service from psychologists, psychiatrists, medical professionals, is required and transportation is provided within a 60 mile radius per week.

Level V  Court Ordered Therapeutic Care
This service is designed for consumers with court ordered sentences to our facility with a higher level of security.  This consumer is considered “High Risk” and appropriate fees will incur.  Additional contracted services may be required for one on one personal consumer services for decreasing the risk of potentially volatile situations.  Twenty-four (24) hour supervision provided for safe management of out-of-control behaviors and medication management.  Individualized treatment plans and activities will be developed to help the consumer master their obstacles in achieving or maintaining court orders and conducted in a manner that is fully integrated into ongoing therapy.  Consumer is subject to consistent reinforcement of consequences for inappropriate behavior.  Daily documentation of progress  is provided to the courts, families and/or guardian.  Supportive correspondence to courts and probation officers for mandated compliances, as needed.  A higher level of consultative and direct service from psychologists, psychiatrists, medical professionals, is required and transportation is provided within a 60 mile radius per week.

Level VI -- “High Risk”
A consumer at risk for serious repercussion due to past behaviors are considered potentially dangerous and volatile in a Care Home environment.  The following are criteria for “High Risk” Consumers:  known and reported physical abuse to care home staff; more than five arrests;  indecent exposure and sex related issues are automatically “High Risk” with Top of the Line.  Additional contracted services may be required for one on one personal consumer services for decreasing the risk of potentially volatile situations.  Twenty-four (24) hour supervision provided for safe management of out-of-control behaviors and medication management. There is a standard “High Risk” fee and is subject to change based on each individual case.

Semi Independent Living

Top of the Line apartments are available for lease to consumers who are compliant with medication management and are independent with activities of daily living i.e. cook, clean, laundry, work, drive, etc.  The opportunity arises for consumers after successfully mastering their obstacles under our therapeutic care to elevate into semi-independent living.  They look forward to an independent living arrangement and the opportunity to become a viable functioning member of society.  We remain a pillar of support as they move out into the community and assume responsibility for their own lives.

We have graduated ten consumers into semi-independent living in the last few years.  Some have acquired employment, married and are living successfully in society.
